Second Death at Prairie House Triggers Lawsuit and Public Warning from Alpha Firm

Tulsa, OK Attorney Donald Smolen II is speaking out after a second resident death has been linked to Prairie House Assisted Living and Memory Care, a facility entrusted with caring for individuals with cognitive impairments and long-term medical needs. The Alpha Firm is now calling for a full-scale investigation into what appears to be a systemic failure in care and supervision.

In this latest case, a woman in her early 60s was suffering from cognitive decline and under “24-hour supervision”, was found naked and disoriented in the facility’s courtyard during a cold Oklahoma winter night, where she remained for hours. She became severely ill following the incident and died ten days later. This is the second known instance of a resident at Prairie House being found after an extended period of time, overnight, in this courtyard. The other known resident was found dead.

“This facility promised safety, dignity, and protection,” said Smolen. “Instead, two residents were able to leave a locked unit without staff noticing, with fatal consequences. This isn’t just tragic, it’s outrageous!”

Smolen is demanding immediate answers:

  • How did two residents escape into a courtyard at night?
  • Where was the supervision?
  • What policies were in place and who failed to enforce them?

“These residents didn’t wander off in a split second. This took time. This took neglect,” Smolen said. “We need to investigate every inch of this facility, its staff, its records, its leadership—and determine how many people may be at risk.”
